Today I applied for a new passport in Bangkok and was confronted at the embassy with the statement from the Dutch embassy that would be required to convert the current visa into the new passport, which is confirmed by the embassy in this statement that it issued passport.

I have been in the Netherlands with my husband since June last year. We would like to go to Portugal on holiday by plane this year. During booking, I was asked for personal details such as those in my Thai passport. When I compared my passport details with that of my ID as provided by the IND, it turned out that the ID shows my maiden name, while my Thai passport shows me with my husband's name. I changed my surname to that of my husband.
So on my passport I'm listed with my husband's name, and on the ID with my own name.
