My name D. 173 cm tall, 63 years and 65 kilos. I have my PSA checked every year from 2013 and often during this period. In 2016 state hospital and 2019 Bangkok hospital, I had a biopsy done, I know, and no cancer was found here.
To take a look at the last few years, the values are 2016 8.27, 2017 7.73, 2018 11.689, 2019 11.56, 2020 89.86, 2021 13.363, 2022 15.776. It keeps fluctuating but it fluctuates a little higher each time.
Now my question is when should I start to worry and how quickly can this metastasize?
I use Firide 1mg or alpha blocker Finax 5mg every other year for 5 year to see if these help, sometimes it goes down and the next year it goes up again. For the last year I have used Regenez 1mg and this has not helped.
I do use 4mg Doza every day to make urinating easier and 1 aspirin because of TIA.
My brother, 74 had a PSA value of 398 last year and metastatic prostate cancer was diagnosed, he has now been cured of this by chemo and bicalutamide and some other medicines.
So that I would like to know your opinion if I should be concerned?

Dear D,
PSA is a very unreliable method of measuring the condition of the prostate. The larger the prostate, the higher the PSA.
If you want a better test, have an MRI of the prostate. This is currently the most reliable non-invasive test.
Firide shrinks the prostate and alpha blockers such as Doza and Finax dilate the urethra, making it easier to urinate.
