Can someone help me fill out an M5 tax form?

Can someone help me fill out the M5 tax form? I am living near Phitsanulok and got married in December 2017.
Unfortunately, I have had a cerebral infarction twice, so I can no longer travel to the Netherlands.

Helped someone before. After all, filling it in is only half an hour's work. And I like work.
But what turned out. The person had no year of statements from the various agencies where income was. Didn't know his wife's social security number. Etc. Etc. Etc. In short, it took a few months to write letters and request the necessary information. After all, write a letter waiting for an answer and then discover there were even more income.
So in the end it wasn't helping with filling out a tax form but putting a neglected administration in order. No thank you after a few months of work.
So HJG what do you mean by help tax form. Do you mean you need an administrator, which can be months of work or someone who has to fill in all the necessary data in a declaration, which is half an hour's work.

When I read your story I can only come to one conclusion: you handled it completely wrong.
I take care of around 30 Model-M returns every year. My customer sends me an e-mail with a “read confirmation” containing his DigiD data. We agree in advance around what time this message will be sent. Half an hour after the read receipt, he changes his password.
In the meantime, I can request sufficient information from the Tax and Customs Administration about the annual income received and from “My government” to find out the registration in the “Register of Non-residents” for the correct date of emigration.
I then request an M form for him/her from the Tax and Customs Administration.
Filling in the M form does not take much time. However, more time is spent on compiling a report with an appendix containing a calculation of the expected outcome for income tax and national insurance contributions. The latter is absolutely necessary because, as long as the M form (a paper declaration) is used, I have not yet experienced that this has been correctly completed in one go by the Tax and Customs Administration/Office Abroad. And if a customer does not know what to expect, he or she will soon accept the later provisional assessment. However, it is not uncommon for the Tax and Customs Administration to be several thousand euros off. €5.000 is more common than €1.000.
But, fair is fair: that happens just as often to the advantage as to the disadvantage of the taxpayer. And if it is to his or her disadvantage, I will immediately submit a request (with a detailed calculation, also of the differences) for revision of the provisional assessment. I also request a postponement of payment. It is not uncommon for this procedure to be repeated 1 or 2 more times.
And so, all in all, it takes quite a bit of time to prepare a paper M-note.
Making such a calculation is quite specialist work. Luckily I automated this.
