Dear Martin,

Am 70 years old, 1,68 m. 63 kg. non smoker/drinker. What are alternatives in Thailand for:

1) Triplaxam 5/1,25/5 mg (blood pressure)
2) Asaflow 80mg
3) Simvastatin 20mg

I still have high blood pressure (average 150/80) Since Triplaxam has 3 ingredients, wouldn't it be better to try another remedy with 1 ingredient that is available in Thailand?

Thank you.

Greeting.

P.

Dear P,

– Asaflow is available here under the name Aspent-M 81, or Aspirin BD 81 mg
– Simvastatin is also called simvastatin here. In addition, there are many preparations with other names that contain the same thing
– We can divide the Triplaxam into: perindopril arginine 5mg, indapamide 1.25mg and amlodipine 5mg. They are all available under the same name.

Combination preparations are easy, but have the disadvantage that you swallow everything at once. Distribution over the day is often better.
We are waiting for such preparations with a release clock. That technology already exists.

My advice:

Aspirin 81 after, or with breakfast.
The simvastatin, although superfluous in my opinion, after breakfast.
Indapamide 1,25 (inpamide, frumeron) at breakfast.
Perindopril around 18.00 pm
Amlodipine 10 half an hour before going to bed.

Then do not measure your blood pressure for two weeks. That does not help.

After two weeks you will know how it stands. If it is still a bit high, first increase the indapamide, in two-week steps to a maximum of 5 mg. Measure in the morning and evening before going to bed. If your blood pressure is good at 130-150/70-90, measure every two weeks and if you are not feeling well.

According to the cardiologist, the values mentioned are not ideal, but they are better to live with. That is often forgotten. Many cardiologists would prefer to treat a heart with nothing around it.

If you do not get your blood pressure in order, you should take something else, but that will be determined by your doctor.

Yours faithfully,

Dr. Maarten
