Thai Covid-19 vaccination passport
The Thai Ministry of Health issues a vaccination passport, as previously published in the Royal Gazette.
This document contains details of your vaccination for international travel. Above is the information about necessary documents and contact offices for those who would like to obtain a vaccination passport or international vaccination certificate.

I obtained the Thai vaccination passport in Chiangrai at the hospital where I also had the vaccination. Costs 50 bath.
Is this passport only available in Bangkok?

Take a good look..right next to Yellow Thai Vaccination Booklet..In Dark Green..
It says that in Elke..Provincial Public Health office such a Yellow booklet is available..
So also In The City/District Where You Stay…
Would be first called by your partner..here in ChiangMai..”Out Of stock”
They can't make it more fun…

Got the booklet a while ago, went pretty smoothly at the Lamphun health office.
Is only issued to vaccinated Thai or foreigner who can also provide a valid passport.
Cost 50 baht, payable upon receipt.
In my opinion, it is not much of the same quality as the international driving license that I once received from the ANWB, easy to copy for malicious parties.
