Can you extend your visa for 60 days by 30 days immediately after arrival in Bangkok? Or do you only have to do this on the last day? Otherwise I would have to travel to Bangkok again from Surin.

You can extend your period of stay at any immigration office. You don't have to go to Bangkok for that.

When it will be allowed depends on the immigration officer. Usually one will say to come back in the last week or 14 days, others will allow it regardless of when you ask.

Waiting until the last day is never a good idea. You never know what could happen that day.
