When you apply for an online Visa STV, you will be asked questions regarding:

A. Alternative State Quarantine (ASQ) or Alternative Hospital Quarantine (AHQ) Reservation Confirmation
B. Documents and payment in regard to accommodation (after 14 days in ASQ)
C. Applicant's spouse (no age specific) or children (age under 20 years old) (I have a girlfriend in Thailand)

If you do not complete these questions, you cannot complete the visa application with a payment. Does anyone know why these questions are needed and what it means exactly?

The STV is a temporary visa that normally expires at the end of September 22. This also means that you can currently only stay in Thailand until the end of September. It is therefore possible that the requirements will not be adjusted for the time being and that they may no longer be issued. It is now a matter of waiting whether the use of STV will be extended, made permanent or whether it will be stopped permanently. I don't know when that will be decided.

The requirements you list are related to corona measures, but are no longer applicable. To know the correct requirements of an STV, you should look here:

Tourism for a longer duration of stay but no longer than 90 days

VISA TYPE: Special Tourist Visa (STV) (90 days stay)

FEE: 70 EUR for single entry (3 months validity)

As you can also read there, the stay is currently limited to September 30, 2022.

“EXTENSION OF STAY

STV visa holders who wish to stay longer must file an application for permission at the Office of Immigration Bureau (https://www.immigration.go.th). The extension of stay (up to 2 times with the maximum of 90 days per each extension but will not exceed 30 September 2022) is solely at the discretion of the Immigration Bureau.”

Applying for an STV with which you can actually only stay until September 30, 22 makes little sense now, I think. And I don't think they will publish it anymore given the short validity. Or you are now waiting for them to possibly extend it, but you do not have that certainty. Or you choose another visa.
