Reader Question: Fit to Fly statement for my Thai fiancee
Dear readers,
My Thai fiancé is leaving next Wednesday from Belgium to Thailand with Emirates. One of the required documents is a FIT TO FLY statement in English from a general practitioner.
Where can I download a sample document to present to the GP?

Where can I apply for or obtain the FTF?
The Fit to Fly document is not an official document, but rather a “draft”. In principle, any type of certificate issued and signed by a doctor, which indicates that the passenger is fit to travel by air (from a certain date and possibly under certain conditions) can be considered and accepted as a Fit to Fly.
Some airlines may make a pre-formatted document available to their customers for their doctor to complete. The same applies to travel assistance companies such as Europ Assistance, which has this document in French and English.

Found a form here in German and the English language that the doctor can provide with a stamp and his signature.
Normally this should be sufficient for proof that you are Fit to Fly.

In addition, if you type Fit to Fly formular for download into google, you will be given options to print out the form yourself, which a doctor can later stamp and caption.

Your own doctor is not allowed to do that, my GP says she is not allowed to do that, called other GPs and they don't do it either, I have now called Medimare, they do it, send i.mail to [email protected] they sent and fill in the form and send it back they check if it's ok then you get it back with stamp costs 60 euros

In the Netherlands, the attending physician (your general practitioner) is not allowed to issue you with a 'Fit to Fly' statement. However, there are known cases where this has happened.
Why GPs other than your GP did not want to issue a 'Fit to Fly' statement is a mystery to me.
I don't know how it is officially arranged in Belgium.

Is the rapid test accepted? I recently read somewhere that it was not accepted. The article about the current situation states that Thailand itself will use rapid tests for cost reasons.

You have to call Medicare 06703697189.
Say you need a fit to fly certificate.
They will send you an online form to fill out.
Are just a few questions.
Send everything back and put a stamp on them.
It's pretty simple. Costs €60

Officially, the GP is not allowed to do this, but some do. It's a very easy way for companies like Medicare to make money: fill out a form and get the F2F statement in the email. Child can do laundry.

Positive: it is very simple to get such an explanation, it is essentially about nothing: no physical examination, nothing. Disadvantage: it's not about anything to me but yes: it has to be because: corona.
It is understandable that Michel needs a Fit to Fly document for his Thai wife and therefore put the question here. Before I left for Thailand I also needed such a statement. My GP hardly knows me because I fortunately never go there, but he does sign the statement. How can he know that I am fit to fly. The Covid19 test had already been done elsewhere with a negative result. Now I read that you can also arrange this online via Medicare for Euro 60,00. I wonder what function this statement has (other than that you need it to fly) if no medical examination takes place at all.

There appears to be a heavenly difference regarding this document between NL and Belgium. When I made my application in Brussels, the embassy sent me a document like this per PDF.
It was nothing. Don't have it anymore but were three sentences. I Doctor in Medicine certificate that my patient is fit to fly on date ….
In Belgium you can just take it to the doctor and he just fills it in and puts his stamp and signature on it. Costs you a regular consultation.
I would send an email to the embassy [email protected] and ask them to send you a document like this.
Please can you send me the fit to fly certificate.
