Dear Martin,

I am a healthy man of 83 years, weigh 78 kilos and am 190 cm tall. I don't use tobacco or alcohol. My blood pressure is 130/80 and I take 15mg rivaroxaban daily as a blood thinner.

My problem is that I have been suffering from a red irritated penis for over 3 months. The tip of my head, about one cm is bright red and the first 3 cm of my penis. When I went to the doctor three months ago I had to keep everything dry, not wash with soap and treat with talcum powder. Because I also had zinc ointment I was able to limit the inflammation quite a bit.

Meanwhile, my palms started to peel invisibly and I got small round red spots on my chest and back. With the exception of my palms, I felt no itching or pain.

Despite the loving treatment of my penis, there was no improvement whatsoever. So I went to the dermatologist who diagnosed a fungal infection. Because I had this for months, I received 10 capsules of Sporal 100 mg as medication. and Clotrimazole cream.

Yesterday, after 14 days returned and told that the cream does not work and even worsens the penis problem. Of my own accord I had switched back to my zinc ointment for the last 3 days. Now have to come back in 2 weeks, no medication and continue with zinc ointment and talcum powder. The dermatologist was a little short-tempered.

All the red spots on my skin have disappeared.

My question to you is whether the healing effect of the Sporal capsules takes some time and whether you know of another healing cream that I can buy at the pharmacy instead of the restrictive effect of the zinc ointment?

Thank you in advance for your advice.

Regards,

J.

Dear J,

If we assume that there really is a fungal infection and it looks very much like it, then it is probably the talcum powder that is perpetuating the irritation. The treatment by the dermatologist seems correct to me at first.

Clotrimazole is also available as a powder. Mix that with the zinc ointment and apply it very thinly. Clean underwear every day and wash well with clear water.

You can also use miconazole foot powder. For all that, it seems like a good lab. test your penis. To this end, they use a cotton swab. Then they can determine whether there is a fungal, bacterial infection or both.

If you have a foreskin, make sure everything underneath is also clean. Don't wait too long to act.

Yours faithfully,

Dr. Maarten
