Liability statement for COVID vaccine

Coincidentally, today 04/10/21 I had to do my 2de Get Pfizer vaccine in Kanchanaburi.

On the first submission I responded that I couldn't remember having to sign anything of liability, just a paper whether I have certain conditions or medication I take.

One of the reactions to this was that the last line of the form, which is hardly readable, does state that by signing the form, you yourself accept the consequences. Or that everyone signed up for it. When people say no, they haven't been paying attention or weren't interested.

That is why I have now also taken a photo of the document to show what it says. It was the same document only it now had 2 at the topde dose. The rest of the text was the same.

Apart from confirming that I have received and understood information about the vaccine, I don't think there is anything about liability there.

The information given, per groups of 6 (sharing the vial together) only concerned that the vaccine from vial nr…. came and we were going to share that with the six of us, the number and that you then had to wait 30 minutes. If you developed a fever later you could first take a paracetamol and if it didn't get better you had to contact the hospital.

So much for the information that was first given in Thai and then in English per group of 6.

I then thought maybe it happened during the registration for the vaccine, but also checked there and there too nothing about liability. Is also only registration of course and not a signed statement. Not that I would really care, because I would have signed it if it was there, but I can't find any specific text about it anywhere in the papers I signed.

I don't often read about something about it, but of course it could always have happened.

All this just for information and to report that there are several versions of those forms in circulation and there are also those that do not specifically state that liability.

    I presume that the different versions of the forms in circulation are due to the way of interpretation of the relevant officials who have drawn them up. The version 'Screening and Consent Form' that Sjaakie had added came from Bangkok Hospital in Pattaya, so you see that each province has made its own version for this document. I spontaneously think of the differences that sometimes exist in government agencies from the different provinces. This is also Thailand, the wind does not blow in the same direction everywhere. 555
