This shows that it is not the same everywhere. Even a medical certificate and it specifies that this must come from the hospital and must not be older than 7 days. Doesn't happen often, but also in Phuket if I'm not mistaken.

The fact that insurance is requested in this case (NON-OA) is very normal and in accordance with official requirements. This is also written at the bottom of the document.

    New to me is point 12. Photo in front of your house with house number.

      It is indeed rather exceptional for a “retired” but you sometimes hear it.

      With a “Thai Marriage” this is more of a fixed requirement and I think this is asked everywhere.
      In Kanchanaburi there must be six different ones. The house number must also be clearly visible on at least one of them.
