I have a question regarding the long stay visa OA. The Thai embassy states, among other things, that it is necessary to submit the following when applying for the visa in question on their website: 1. A personal data form, 2. An English extract from the Birth registration, 3. An English extract from the Register of population. Numbers 2 and 3 seem to me to be an extract from the birth register and population register respectively. It is not clear what needs to be discussed with “A personal data form”.
