I read yesterday in the Thailand visa question No. 330 /21 from the questioner Alain, that he is talking about filling out a TM-7 form.
Is this necessary when applying for a 60-day tourist visa? How do I get this form and do I have to upload it?
Do you know if people also ask for a bank statement with an application for 60 days and do I have to upload that as well?

The “TM 7 – Application for extension of temporary stay in the kingdom” is a form to request an extension of your stay in Thailand. Doesn't it say anywhere that you need that to apply for your visa.

For a tourist visa it says that you have to charge one of the following.
Financial evidence eg bank statement, proof of earnings, sponsorship letter
E-Visa categories and required documents – สถานเอกอัครราชทูตณ กรุงเฮก (thaiembassy.org)
