Is it possible, with a multi-entry visa where you have to make a border run every 3 months, to do this at an immigration office in eg udon and obtain the new 3 months there?

No.

You can only obtain a new period of stay by making a new entry with that visa. You can, however, extend by a year, or once by 60 days if married to a Thai. The period of stay is expressed in days and in this case it is 90 days. That is not the same as 3 months.
