I'm leaving for Thailand in October, to Bangkok. Can I continue to Phuket a few hours later without having to quarantine in Bangkok?

You can land at BKK and travel on to Phuket provided your luggage is checked through to Phuket. The flights on your ticket must be consecutive, so you cannot land in BKK, collect your bags and then take a domestic flight.
To the best of my knowledge there is still no transit option to Phuket on Suvarnabhumi, so you will have to fly directly to Phuket. An airplane that, coming from abroad, first lands in Bangkok and where you just stay on board, is also possible.

There are two options.
One is indeed a direct flight to Phulet. Thai airways flies from Frankfurt I think but could also be Munich, direct to Phuket. There will be other airlines flying direct to Phuket from another country.
Then you can buy two tickets. An amsterdam (or Brussels) or …. And another ticket from this place, so Frankfurt directly to Phuket.
The other option is a regular, so accepted transfer in Bangkok.
From Bangkok, Bangkok Air flies a connecting flight to Phuket three times a day.
BUT there is a rule of the game. This means that you book your two flights in the same booking process and can therefore see both flights on your confirmation as a connecting flight.
My favorite example is the following. Enter: klm.nl and enter: departure: Amsterdam and arrival: Phuket. You will then be given different options. I'll tell you what our experience was last month. We had the tickets since last year with a transfer in Bangkok. The application for the COE required a 15-day quarantine in Bangkok, for Phuket we had to buy new tickets. And then say something is not true, Google expert!

A: From August 1, 2021, you can fly to Samui on a direct international flight or international flight via Bangkok and connect with the approved twice daily 'sealed route' flights operated by Bangkok Airways between Bangkok and Samui as per the details below.

You are only allowed to fly direct to Phuket. I just left Phuket. Had a flight Emirates Dubai Phuket this is allowed. Passed without any problems. PCR test on arrival and 2 more times on Phuket. Then rented a car to Bangkok where I parked my car. From there to cheek Sam Mo Isaan and no more testing there as I have proof of being vaccinated twice in the yellow book. This was known in Wang Sam Mo. So once you left Phuket you didn't have to test anywhere and you have free travel in Thailand.
PS even got the phone number of the doctor at the hospital with the message if anyone in the village had problems with my arrival they could call her

yes, see my response above. In the example mentioned there is a “guided” transfer from KLM from Amsterdam to Bangkok and then, guided from Bangkok to Phuket with Bangkok air.

As you yourself show: possibly for Samui, not for Phuket.
