We travel to Thailand in July 2018 with our children (then 11 and 13 years old). We do not wish to use public transport because it is too time-consuming, but prefer to rent a car/minibus with driver each time. We did this before in Bali where it was quite easy.
Our route runs from Bangkok to Kanchanaburi, to Phitsanulok, to Sukhothai, to Lampang, to Chiang Rai and Chiang Mai.
Is it easy to find such a service in all these cities or is it better to make an appointment with an agency that drives us around all the time. Or with a private person? Do you have to provide them with food and sleeping places throughout the entire route?

my experience is that those drivers sleep in the minibus (immediately also security for the car) but that can all be CLEARLY agreed in advance and also in terms of food for the driver.
you can find food everywhere and for them at a mini price and for that you can also agree on a daily budget ...
Of course, something like this also depends on your budget, but if you don't really have to look at a euro, then this can be better than expected and indeed you save a lot of time with it.
the problem is where to look for such a person when you arrive in BKK…
For example, if you are in Pattaya, you can ask any taxi driver (they all know someone) but in a big city like BKK not that easy, but maybe worth a try.
if they make their price and you have no experience with Thailand then immediately offer half even if you think it is an excellent price and you can still go up…

Rent a minibus with driver. = 2.000 bath/day. Do I also have to pay for the fuel?
We already have the planning in place in that regard. Almost the same route in 9 days. Do we want to do with minibus with driver via greenwoodtravel. Costs 2750 baht per day including driver's overnight stay. Diesel costs are added, thought € 1 per liter and the empty return trip to Bangkok for which they ask 5800 Bath. I find the latter a bit pricey. We are going with 4 adults. In Chiang Mai we say goodbye to the minibus and continue our journey with 6 days Chiang Mai, 4 days Pattaya, 6 days Koh Chang and 1 night at the airport. From Chiang Mai we fly to Pattaya and the transfers also go by mini-van.
So you find 148 euros (5800 B) all inclusive for a distance of 700 km. “a bit pricey”.
I always pay 90 euros (3500 B) from Subernabuhmi to Trat, a distance of 300 km, including tip. This is a standard price.
If you factor in depreciation, gas/diesel and highway costs (even in Thailand cars are expensive) there isn't really much left for the driver — and then Greenwood also has to earn something.
So I think it's a very fair price.
For comparison:
For Schiphol – Amsterdam-west (16 km, 20 minutes) I pay 42 euros, ex tip.
Well, I know that Thai people earn a lot less than we do, but they don't want to bite the bullet here either...
Standard price for renting a minibus / van is 2000 baht for a day regardless of the km, and you do pay gas / diesel in addition. Well for 700 km, 1 day's drive is 2000 + 1500 for fuel = 3500. Anything more is too much.
The price of the rent, 2000 per day, therefore also includes wages and depreciation, etc., excluding fuel as I mentioned.
And this is the price for Thai people and other initiates.
Look if you pay 3500 baht for 300 km is your own choice, but too much. That's why they like to drive those ignorant tourists around and earn 2 as much as normal.
Ger, I realized afterwards that the driver also has to go back…. so it is thick 600 km, including tip. (Whether he manages to pick someone up on the way or not). So yes, you're right. Does NOT take away the fact that it costs at least 3300 baht to get from Subarnahabum to Trat!!

We have very good experience with renting a van (for 10 people). Our driver knows the region where you want to go. He is very familiar with this area, arranges hotels, etc. However, he only speaks Thai, but that doesn't bother us because my wife is Thai.
We can always have a good driver if we want to cover longer distances. Cost 500 THB per day + food and drinks. On the way there is a stop at a 7/11 where a simple snack is eaten. We do have the habit of inviting him to supper in the evening. Usually our own pick up is used as there are only 2 of us. Personally, I would recommend renting a minibus because the comfort is greater than a normal car: you can take a nap and there is more than enough space for your stuff. As Jan rightly points out: the driver sleeps in the minibus. If you are interested: my e-mail address is known to the editors.
Addendum: the price for renting the minivan, parking, toll and fuel must of course be added.
My brother-in-law does this for his job. Has a very nice bus with plenty of room for everyone. Speaks Dutch and English (and Thai of course).
Costs 1800 baht per day, excluding diesel.

The four of us also made a tour about 10 years ago. The costs were then 25 euros per day. The driver slept in the van and took care of his own food, even when we asked if he would join us for dinner. However, we had to pay for refueling every time, when we went to Laos for 3 days, he waited at the border, which also costs 25 euros per day. I think it will cost double now.

The first took us to pattaya as a taxi with an suv and was satisfied with it. The second one my friend tried this year to go from BKK to Ayutthaya by minibus and were satisfied.
Maybe ask for a price quote at both by email with the things you wish to visit.
Near the historical site of sukhothai (1,5 km) I used a beautiful hotel this year for 1250 bath/2 persons and breakfast included. You can also rent bicycles for 50 bath / pp per day and in the evening for free to drive to a restaurant Sinvana 750 meters away. The hotel is Scent of Sukhothai. There is also a clean swimming pool and rooms were very clean with bathroom air con and pergola overlooking the garden.

Last year we arranged a minivan with driver but no petrol included for 7 days through Eric van Viengtravel. Those guys have been in Thailand for 35 years and know exactly what they are doing. The driver drove neatly and knew a lot about the sights and arranged for his own place to sleep. We could also call the office if communication failed, but that was not necessary.
We are going to Thailand again next year and then again for several days the same driver Nun.

I have very good experiences with rushtaxi.net (or google mr. Rush, Thailand). Speaks perfect English, very good and quite new fleet, actually the first Thai taxi driver I felt safe with in 10 years! Drive calmly and in anticipation.
Maybe not the cheapest (although you can just send it to him by email), but in my opinion cheap is expensive, especially in Thailand where the traffic is so dangerous.
As for sleeping: if you rent a sedan, the driver sleeps in a bed – also in your interest, to have a well-rested driver!
When renting a minibus (with more space) I don't know: you can ask.
