Dear Martin,

Thanks for your reply. I don't have to worry about dirty hands. The practicioner was not inferior to a serious nurse, and knew perfectly how to put on and take off rubber gloves. Actually, I was more afraid of an infection “from within”.

The general question I asked is actually a bit more personal. Last time a starting ejaculation was stopped, actually by a misunderstanding and I'm sure a firm retro ejaculation took place. Now I have a UTI due to a multi resistant E Coli that is sometimes active sometimes not and two days after it happened my left testicle was the size of a large chicken egg. Painful at first but gradually less.

An Ultrasound showed that the epidydimes was quite swollen, also feels like a hard “ring”. I consulted two local urologists and one of them wanted to operate immediately. My own conservative approach pays off. To be on the safe side (because I doubt an infection) I took two weeks of ciprofloxacin (2 x 500 mgr) for a week. No pain, no fever.

The swelling is almost gone, epididimis remains very swollen. Not warm. nothing "pulses" according to ultrasound increased blood flow which seems logical. Sperm on a urine test strip shows no leukocytes, nor nitrite, or the like. Urine is also infection free. So I think it's all okay.

The absorption of the ejaculate by the body naturally takes some time.

I'll keep an eye on it. Think it will work itself out. Please comment if you disagree with this story.

Regards,

T.

Dear T,

This is indeed a more serious question. If you have a resistant E. Coli, there is indeed a chance that they are hiding in the prostate, where 99,9% of the fluid comes from. The seed (sperm) comes out of the scrotum. Together we call that ejaculate. Ejaculate on a urine test strip does not work. That has to do with the composition. If you want to have your ejaculate checked, it must be delivered fresh and warm to a laboratory for culture.

Maybe you can bring your Karsai Therapist with you to give you an uninhibited massage. In the lab. Do they have a van to collect your semen?

A retrograde ejaculation is nothing special and is absolutely not dangerous. The ejaculate then ends up in the bladder and is then passed out together with the urine. So the body does not absorb it.

You probably had an epididymitis with unknown bacteria. Usually the cause is a venereal disease, such as Chlamydia or Gonorrhea, often also unknown. E.Coli is also possible.

Increased blood flow is normal with an infection. Antibiotics are the right treatment, but if you don't know what bacteria it is and don't have an antibiogram, it becomes blindfolded.

Yours faithfully,

Dr. Maarten
